The implementation of EU MDR (EU 2017/745) continues to deepen, with harmonised standards and industry technical benchmarks evolving rapidly. Major updates to CE regulatory documents have recently been released, covering quality management, risk management, usability, clinical evaluation, biocompatibility, labelling and instructions for use, MDCG/MEDDEV guidance documents and EU regulations. Among these, newly released standards including ISO 14155:2026, ISO 10993‑3/6/7:2026 and ISO 2859-1:2026 will directly impact enterprises’ quality management systems, test protocols, clinical investigations and technical documentation drafting. This is highly relevant for all medical device manufacturers planning or undergoing CE certification.

一、Core Revisions of the Three New 2026 Standards

(一)ISO 14155:2026|Groundbreaking Revision of Medical Device Clinical GCP
As the core GCP standard for clinical investigations under MDR, ISO 14155:2026 has undergone substantial technical revisions, imposing stricter mandatory requirements on clinical protocol design, risk control, organisational structure and the full digital clinical workflow:
Dual-track risk management:A clear distinction is made between subject risk and data integrity risk. Device-related risks shall be managed in accordance with ISO 14971; independent risk assessment requirements are added for non-routine clinical operations arising from clinical investigations.
CEC/DMC formally incorporated into standard requirements:The Clinical Events Committee (CEC) and Data Monitoring Committee (DMC) are included in the standard, requiring independence, blind state control and written charters. If such committees are not established, the manufacturer must provide sufficient written justification.
Introduction of the Estimand framework:Aligned with ICH E9 (R1), it clearly defines treatment objectives, target populations, clinical endpoints and intercurrent events, leading to more rigorous logic for clinical endpoint design.
Rigid inclusion and exclusion criteria:Protocol deviations shall not be used to handle out-of-range enrolment. Any adjustments must go through full clinical protocol amendments and obtain ethics and regulatory approval, prohibiting arbitrary relaxation of subject selection criteria during trials.
Dual classification of adverse events:Classification and assessment are performed based on two dimensions: the nature of the event itself and its correlation with device defects, while structured exemption reporting scenarios are defined.
Embrace DCT (Decentralised Clinical Trial) digital clinical models:The standard formally recognises eConsent, remote monitoring, EDC electronic data capture and other decentralised clinical trial models, providing a standard basis for digital clinical work.

Corporate Action Tips:For ongoing or planned clinical investigation projects, assess the impact of the new standard on protocols, SOPs and CRO cooperation agreements. For completed clinical investigations, when compiling clinical evidence reports, evaluate gaps between old and new standards and add scientific justification.

(二)ISO 10993‑3 /6 /7:2026|Upgrades to Three Key Biocompatibility Standards
The ISO 10993 series forms the cornerstone of medical device biological safety. Three key standards were updated in 2026 in a single release, directly changing the logic for toxicological evaluation, local effects after implantation and EO sterilisation residue testing.

ISO 10993-3:2026 Evaluation of genotoxicity, carcinogenicity and reproductive and developmental toxicity
Updates the complete evaluation methodology for genotoxicity, carcinogenicity, reproductive and developmental toxicity. Its risk framework aligns with ISO10993-1:2025, and revisions are made to the selection of toxicology test batteries and result determination logic.

ISO 10993-6:2026 Tests for local effects after implantation
Adds Annex E for test methods for devices contacting peripheral nerve tissue and Annex G for histopathological microscopic evaluation; adds rules for control material selection and guidance for small-sized specimens/test coupons; expands requirements for lymph node assessment; updates the full set of pathological terminology, exerting a huge impact on implantable devices (orthopaedics, implantable consumables).

ISO 10993-7:2026 Ethylene oxide sterilisation residues
Shifts from traditional fixed limit values to a risk-based calculation formula model; distinguishes special populations including adults, neonates and low-birth-weight infants; updates residue determination guidelines incorporating modern analytical techniques; clarifies product release assessment logic. Important changes have occurred in residue calculation and release judgement for EO-sterilised products.

Corporate Action Tips:Manufacturers of implantable and EO-sterilised products shall prioritise gap analysis. Old test reports cannot be used indefinitely; it is necessary to assess whether old test reports meet the state-of-the-art requirements of the new standard. Biocompatibility reports within technical documentation shall update cited standard versions.

(三)ISO 2859‑1:2026 Major Revision of Attribute Sampling Inspection Procedures
This widely adopted AQL sampling standard for manufacturing has released a new version:
  • Skip-lot sampling procedures are newly added;
  • Clarifies requirements for production qualification and product eligibility;
  • Deletes the original OC curve graphs and provides calculation methods for OC curves instead.
Impact: SOPs and inspection specifications for incoming material inspection and finished product release inspection require synchronous revision, together with quality record forms.
二、Full Scope of Updated Regulations

The updated document list is divided into five modules. Enterprises shall update their internal external document registers accordingly to avoid expired versions identified during system audits and Notified Body audits.
General Medical Device Basic Standards:EN ISO13485, EN ISO14971, ISO/TR24971, EN62366-1 usability engineering, etc.
General Product Standards: ISO14155 Clinical GCP, ISO16269-6 statistical tolerance intervals;
Full Biocompatibility ISO10993 Series:Includes 2025 and 2026 new editions and amendments (AMD). Note: USP/EP/JP/KP pharmacopoeia standards for pharmaceutical packaging materials are not CE harmonised standards. They may only be cited conditionally for pre-filled syringes and drug container products, and cannot replace ISO10993-18 chemical characterisation for general medical devices.
Labelling and Instructions for Use Standards:EN ISO15223-1:2021/A1:2026 Symbols, EN ISO20417 Information supplied by the manufacturer;
Guidance & Legislation:MEDDEV series, full set of MDCG guidance on clinical evaluation, PMCF and device classification; EU MDR Regulation (EU 2017/745).

Important Reminder:Pharmacopoeia packaging material standards (USP661, EP3.2.2, etc.) only apply to scenarios where devices also serve as drug containers and are in direct contact with medicinal products. For biocompatibility evaluation of general medical devices, the ISO10993 series shall be prioritised and pharmacopoeia standards cannot be used as a direct substitute.
三、Implementation Action Checklist for Medical Device Manufacturers

Update External Document Register:Upgrade the controlled external document list, identify all revised standards, recall old printed and electronic versions, release new documents and complete training.

Gap Analysis:Identify affected standards by product category:
  • Clinical investigation projects: Focus assessment on ISO14155:2026;
  • Implantable devices: Focus assessment on ISO10993-3, -6;
  • EO sterilised products: Focus assessment on ISO10993-7;
  • Production & inspection departments: Update SOPs related to ISO2859-1 sampling.

Technical Documentation Review:Verify technical documentation (TD), risk management files, biological assessment reports and Clinical Evaluation Reports (CER). Update cited standard versions. If old test / clinical data is continued to be used, scientific justification shall be supplemented.

System SOP Revision:Revise SOPs related to quality management, risk management, clinical activities, biocompatibility assessment, inspection and document control as required.

Personnel Training:Quality, regulatory, R&D, clinical and test engineers shall learn the changes in new standards to align internal understanding.

Tip:The release of a new ISO standard does not mean it immediately becomes an EU official harmonised standard. Even if not yet listed in the EU Official Journal (OJEU), Notified Bodies will treat new ISO standards as industry state-of-the-art. Technical documentation must fully consider changes brought by the new editions.
四、Conclusion

The core of MDR compliance is to keep pace with the iteration of international standards. Many non-conformities identified during certification audits stem from ineffective control of external document registers and continued citation of obsolete standard versions. Manufacturers are recommended to establish a regular standard monitoring mechanism to timely identify standard revisions and avoid pitfalls in registration and system audits.For further information, please contact Wiselink Group!
